COPD stands for chronic obstructive pulmonary disease. It is a disease that develops over time and causes obstruction of the airways in your lungs, as well as limits the flow of air both to and from your lungs.
COPD is the third-leading cause of death in the United States, preceded by only heart disease and cancer. It kills over 126,000 Americans annually. Historically, COPD has occurred more frequently in men; however, since 2000, more women have died each year from the disease than men.
Living with COPD can be challenging, as the disease dramatically impacts your daily life. Often causing severe disability, a diagnosis of COPD can lead to feelings of hopelessness and helplessness. The disease is progressive and will impact physical activity and social interactions.
We have a chronic disease management program that can help you improve your quality of life and take back your independence.
You need to know all you can about COPD- smoking cessation, nutrition and exercise, pulmonary rehabilitation, online resources, inhaler training and techniques, oxygen therapy, travelling with oxygen, alpha 1 testing.
Lung Cancer Screening
The USPSTF recommends annual screening for lung cancer with low-dose computed tomography in adults aged 55 to 80 years who have a 30 pack-year smoking history and currently smoke or have quit within the past 15 years…
Screening should be discontinued once a person has not smoked for 15 years or develops a health problem that substantially limits life expectancy or the ability or willingness to have curative lung surgery. (B recommendation)

Our COPD disease management includes:

Monthly Friday morning COPD social. A 1- to 1.5-hour group education session conducted monthly- “COPD AT THE POINTE”

The patient education sessions which include:

General information about COPD.

Direct observation of inhaler techniques.

A review and adjustment of outpatient COPD medications.

Smoking cessation counseling.
-
Recommendations concerning influenza and pneumococcal vaccinations.

Encouragement of regular exercise.
-
Instruction in hand hygiene.

You will receive an individualized written action plan.

Patients are to be in possession of action plan medications at all times and are to have refill prescriptions immediately upon initiating the action plan, if indicated.

Our case manager will make made monthly phone calls to you to address your COPD management.

We encourage patients to call the case manager during regular working hours if they took action plan medications or if they visit an urgent care or emergency room or if they had questions relating to their medical care.
